Consumer Directed Service Design and Delivery

Achievement Center – Easterseals takes an individualized approach to consumer service planning and delivery. AC-ES planning and service delivery procedures are designed to integrate the consumer into the entire process. Throughout your program at the Center, you will be asked to participate in planning your Individualized Evaluation Plan, Individualized Program Plan, Individualized Work Center Program Plan, and Individualized Employment Plan. Your progress in the program will be constantly evaluated by your supervisors or Program Manager if receiving in-house services. There will be monthly staff meetings where you, the Coordinator of Vocational Services, supervisors, and Referral Counselor will discuss your program and make any needed changes. All participants will be asked to participate in satisfaction surveys. Other stakeholders input is important to the planning and service delivery procedures of the AC-ES and is solicited through surveys, family involvement, and one on one contact. Through your consent, family or other individuals involved with you and your program may be asked to assist us by completing satisfaction surveys.
